What is an Inquest Report?
Inquest reports are prepared by independent expert witnesses to assist a Coroner’s Court in investigating the circumstances surrounding a death.
It provides an impartial, evidence-based medical opinion to help the Coroner understand the medical aspects of the case, determine the likely cause of death, and establish whether any clinical or systemic factors contributed to the outcome.
Our Inquest reports are prepared in accordance with Coroners (Inquests) Rules, ensuring independence, clarity, and compliance with all legal requirements.
An Inquest report may be required when; a death occurs in hospital or in another healthcare setting, there are concerns about the standard of care provided before death, the cause of death is disputed or unclear, the death is unexpected or occurred in state detention.

Our Inquest reports are charged on an hourly basis and typically include:

We also provide a fee estimate, so there are no surprises. This estimation will be dependent on the volume of records, the number of issues that need to be addressed, and the general complexity of the case.
The hourly rate is set by the individual expert and reflects their specific area of expertise, the length of their clinical career, and their level of experience in preparing expert witness reports.
