Hannah McKelvie is a Registered Adult Nurse and Independent Nurse Prescriber with over 23 years of experience in community nursing, district nursing, palliative care, long-term condition management and clinical leadership. She has held senior operational and strategic leadership roles across NHS community services and the hospice sector, combining extensive frontline clinical practice with expertise in governance, quality improvement, patient safety and service development.
Hannah currently serves as Clinical Operations and Quality Lead where she has overall responsibility for multiple clinical services including Hospice at Home, Day Hospice, counselling services and children's services. She leads clinical governance, incident management, audit, quality improvement and strategic service development whilst continuing to support patients and clinical teams directly.
Prior to this, Hannah spent over a decade leading NHS district nursing teams as Professional Nurse Lead and Community Matron. She has extensive expertise in community nursing standards, virtual wards, long-term condition management, admission avoidance, integrated care, prescribing, multidisciplinary working and the delivery of care within patients' own homes.
Her leadership experience includes investigation of clinical incidents, governance reporting, clinical audit, risk management, quality assurance and implementation of system-wide service improvements. She has developed governance dashboards, led organisational audit programmes and supported strategic clinical transformation across community healthcare services.
