Professional Profile
Nicola Smart is a Registered Adult Nurse, Specialist District Nurse and Independent Nurse Prescriber with over 13 years of post-registration experience across acute and community healthcare. She currently practises as a Community Matron within a busy NHS Trust, providing autonomous assessment, diagnosis and treatment for patients with complex healthcare needs whilst leading community-based initiatives to reduce unnecessary hospital admissions.
Nicola has extensive expertise in district nursing, urgent community response, long-term condition management, end of life care, palliative care, advanced health assessment and Continuing Healthcare (CHC) assessments. As an Independent Prescriber, she provides timely community-based treatment for acutely unwell patients, supporting rapid intervention and avoiding unnecessary hospital attendance wherever clinically appropriate.
Throughout her community nursing career, Nicola has managed highly complex patient caseloads, coordinated multidisciplinary care, undertaken pressure ulcer reviews, conducted clinical audits and led incident review meetings. She has significant experience in wound care, enteral feeding, PEG management, intravenous therapy, catheter care, pressure ulcer prevention and community-based management of patients with multiple long-term conditions.
Prior to specialising in community nursing, Nicola worked within a busy Accident and Emergency Department where she gained valuable experience in acute patient assessment, triage, sepsis recognition, safeguarding and emergency care across adult and paediatric patient groups.
